SEMVMA Academy Application

The SEMVMA Academy recognizes individuals who document exemplary pursuit of continued professional development. Recognized members have their names displayed and available to members and the public on the SEMVMA website, their names published in the newsletter, their efforts recognized at the annual Membership Celebration, a certificate, and access to purchase a plaque honoring their commitment to advancing veterinary medicine.

Requirements for membership: 

1.  Applicant must be a SEMVMA member in good standing and have completed a minimum of fifty (50) hours of continuing education (CE) from January 1 to December 31, 2025 (see note below). Applications are evaluated by the SEMVMA Academy’s Membership Committee.   Certification must be included with your application.

2.  Applicant must be a member of the SEMVMA for that calendar year.

3.  Membership in the Academy is free; please circle one of the following: 

a. Certificate suitable for framing    FREE

b. Wall plaque, if desired (first year) $215.00

c. Year plates for plaque (perpetual) $15.00

4.  Return this completed application and a check made payable to SEMVMA prior to February 13, 2026.
